During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 09/08/2025 and completed on 09/17/2025. The on-site inspection began at 10:00 a.m. and ended at 11:10 a.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 staff. The inspector reviewed 5 children?s records on-site and 3 staff records electronically on 09/17/2025. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4Medication shall be kept in a locked place using a safe locking method that prevents access by children.
Aspirin, non-aspirin pain reliever, and antacids were located in an unlocked first aid kit hanging on the wall in the hallway.
The center shall implement a monthly practice evacuation drill.
The was no documentation of a monthly practice evacuation drill for August 2025.
A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.
Required numbers were not posted.
The records for Child 1, Child 2, Child 3, Child 4 and Child 5 enrolled over three weeks, were each missing several of the required components.
