Inspector Notes

An unannounced inspection was initiated and completed on 09/08/2025 in response to a complaint received by the licensing office on 09/02/2025 relating to allegations of administration. The inspector reviewed 5 children?s records on-site. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Centers shall be in compliance with the terms of the current license issued by the department.
The provider was licensed to serve children ages 3 years through 12 years old. For approximately three weeks, the provider had children in care under the age of 3 years.
Plan of Correction: Provider has requested a modification to the license to change the age of children.
