Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 20, 2025 and completed on March 20, 2025. The on-site inspection began at 1:35pm and ended at 2:20pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 4 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (4/11/2025). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment. Staff #2 submitted a tuberculosis screening that was not within 30 days of employment.
Plan of Correction: Appointment has been made for updated TB screening.
Standard 8VAC20-780-245-L
There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. The staff present during the inspection did not have daily health observation training documented in their record.
Plan of Correction: Licensing sent the link for the trainings and all employees have started the training.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There was a bottle of hand sanitizer on a shelf with the label ?keep away from children? unlocked and accessible to children in the 2- and 3-year-old room
Plan of Correction: Hand sanitizer has been put into the adult bathroom and locked in the cabinet.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Name and phone number of the child's physician and the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The record for child #1 did not contain addresses for both emergency contacts. The record for child #2 did not contain the physician?s name and phone number and did not list two emergency contacts.
Plan of Correction: Both children's records have been updated to include all necessary information.
