Inspector Notes
An announced, on-site initial inspection was initiated on 11/07/2024 and completed on 11/07/2024. The on-site inspection began at 9:30 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was eight. The inspector reviewed 2 children's records and 2 caregiver/household member records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe, and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin.
The backyard play area contained multiple hazardous safety conditions including, but not limited to: Assessable and broken household items with sharp points, rust, splintered wood and standing water.
The indoor areas of the home, where children were in care, contained heavy pieces of furniture that were not secured to the wall and a crib placed directly beside a large potted plant.
Plan of Correction: The provider stated that all indoor and outdoor safety hazards will be removed, repaired and secured by November 23, 2024. Additionally, the crib will be moved away from the plant.
Standard 8VAC20-800-320-G
Wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider.
The provider stated that an inspection of the fireplace chimney had not been conducted.
Plan of Correction: The provider stated that an inspection will be completed for the fireplace chimney.
Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. Cordless or cell phones may be used in addition to the landline telephone.
The provider stated there was no landline telephone in the home.
Plan of Correction: The provider stated that a landline phone will be obtained.
Standard 8VAC20-800-50-A
The family day home shall keep a written record of children in attendance each day.
Child 1 was not listed on the attendance sheet for the family day home. Child 1 was in care when the inspector arrived at 9:30 am.
Plan of Correction: The provider stated that children will be added to the attendance sheet upon arrival.
Standard 8VAC20-800-530-B
Cribs when being used by infants shall have a tight-fitting bottom cover.
Child 1 was observed napping in a crib with a loose fitting crib sheet.
Plan of Correction: The provider stated that a tight fitting crib sheet will be obtained.
Standard 8VAC20-800-60-A
The provider shall maintain an up-to-date record at the family day home for each enrolled child.
1. There was no enrollment record for child 1 who was in care during the inspection. Child 1 has been in care for approximately 4 months.
2. The enrollment record for child 2 lacked the following required elements: a. Name and policy number of the child's medical insurance.
3. The enrollment record for child 3 lacked the following required elements: a. Name and policy number of the child's medical insurance; b. Name, office address and telephone number of the child's physician.
Plan of Correction: The provider stated that complete and current records will be obtained for child 1, 2 and 3.
Standard 8VAC20-800-680-A
Caregivers shall wash their hands with liquid soap and warm running water after diapering a child.
The provider did not wash her hands with soap and warm running water after changing the diaper of child 2 at approximately 10:41 AM.
Plan of Correction: The provider stated that hands will be washed with soap and water after each diaper change going forward.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.
The provider did not wash the hands of child 2 after a diaper change at approximately 10:41 am.
Plan of Correction: The provider stated that children's hands will be washed with soap and water going forward after a diaper change.
Standard 8VAC20-800-690-E
Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
The provider stated she did not have any step down type of trash can for disposal of diapers. The diaper pail observed in use was hand generated.
Plan of Correction: The provider stated that a step down diaper pail/disposal system will be obtained.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided in writing the following information:
1. There was no written parent agreement on file for child 1 who was in care during the inspection.
2. The written parent agreement for child 2 and child 3, both in care during the inspection, lacked the following required information:
a. Policies for administration of medication;
b. Provisions of the family day home's emergency preparedness and response plan.
3. The written parent agreement for child 2 lacked the following required information:
a. Address of the website of the department - VDOE;
b. Requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease;
c. Requirement for paid caregivers to report suspected child abuse or neglect;
d. Custodial parent's right to be admitted to the family day home any time the child is in care;
e. Amount of time per week than an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider; f. Policies for termination of care.
Plan of Correction: The provider stated that a complete parent agreement will be provided and signed by each parent.
Standard 8VAC20-800-730-D
Medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.
The mirrored wall storage cabinet in the children's bathroom was unlocked. The cabinet contained multiple medications for adult members of the family day home.
Plan of Correction: The provider stated that all medication will be locked up.
Standard 8VAC20-800-760-A-1
A first aid kit that contains at a minimum: Adhesive bandages, assorted sizes.
The first aid kit lacked assorted sizes of adhesive bandages.
Plan of Correction: The provider stated that assorted sizes of band aids will be obtained.

A working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.
The provider stated she did not have a working portable battery-operated weather band radio.
Plan of Correction: The provider stated that a weather band battery operated radio will be purchased.

The following telephone numbers shall be posted in a visible area close to the telephone: The responsible person for emergency backup care.
The phone number for the individual designated as the emergency backup was not posted in the family day home.
Plan of Correction: The provider stated that the contact for the emergency backup individual will be posted near the other emergency phone numbers.
Standard 8VAC20-800-800-A
The family day home shall have a written emergency preparedness and response plan that contains all of the required elements.
The written emergency preparedness plan for the family day home lacked the following:
a. Methods to alert caregivers and emergency responders;
b. Method of communication with parents and emergency responders after the evacuation;
c. Secondary evacuation routes;
d. Shelter-in-place procedures were incomplete to include relocation, methods of communications and securing of essential documents and supplies.
Plan of Correction: The provider stated that the written emergency plan will be completed to include all missing information.
Standard 8VAC20-800-890
When food is brought from home, the food container shall be clearly labeled in a way that identifies the owner.
A cup of flavored water was not labeled to identify child 3.
Plan of Correction: The provider stated that children's water bottles and cups will be labeled.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.
Child 1 lacks documentation of being adequately immunized. Child 1 has been in care for approximately 4 months and was in care on the date of the inspection.
Plan of Correction: The provider stated that the parent of child 1 will be asked to provide an immunization record.
