During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 02/24/2025 and completed on 02/24/2025. The on-site inspection began at 9:50 am and ended at 11:35 am. The inspector reviewed compliance in the areas listed above. There were 4 children present with the provider; the point total was 12. The inspector reviewed 4 children's records on-site.

Standard 22.1-289.011-F
The inspection documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The inspection summary and violation notice from the most recent licensing inspection were not posted in the FDH.
Plan of Correction: Inspection reports will be posted.
Standard 22.1-289.057-A
Each child day program that is licensed pursuant to this chapter and any program described in subdivision A 4, B 1, or B 5 of ? 22.1-289.030 that serves preschool-age children shall develop and implement a plan to test potable water from sources identified by the U.S. Environmental Protection Agency as high priority, including bubbler-style and cooler-style drinking fountains, kitchen taps, classroom combination sinks and drinking fountains, home economics room sinks, teacher's lounge sinks, nurse's office sinks, classroom sinks in special education classrooms, and sinks known to be or visibly used for consumption.
The provider stated that the potable water inside of the home had not been tested.
Plan of Correction: The provider stated she would review information on the VDOE website and find a lab to test the water in the FDH.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
This is no documentation of a physical exam on file for child 1 who has been in care for approximately 60 days.
Plan of Correction: The parent of child 1 will be asked to provide a current physical exam.
Standard 8VAC20-800-240-A
Repeat Violation
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition.
A stand up fan and upright vacuum cleaner were stored in areas of the child care room accessible to children. A metal dismantled garden trellis was observed in the backyard play area on the ground. Additionally, a tall box containing a backyard play structure/swing was stored on the deck beside the door used for entry and exit to the backyard play area.
Plan of Correction: The provider stated that the box and metal trellis will be removed. The fan and vacuum cleaner will be put away when children are in care.
Standard 8VAC20-800-280
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants and deodorizers shall be in areas inaccessible to children.
A bottle of bleach was observed on the floor in the kitchen. Children were observed in the kitchen during the inspection.
Multiple chemicals and cleaning agents/beauty products were observed in the unlocked bathroom cabinet under the sink in the children's bathroom. Additionally, a spray can of deodorizer was observed on the sink counter in this bathroom.
Plan of Correction: The provider locked the cabinet in the children's bathroom during the inspection. Bleach and air freshener will be removed to an area inaccessible to children.
Standard 8VAC20-800-290
Sharp kitchen utensils and other sharp objects shall be inaccessible to children unless being used by the caregiver or with children under close supervision.
A sharp kitchen knife was observed on the counter in the kitchen. This knife was accessible to a child observed in the kitchen twice during the inspection.
Plan of Correction: The knife will be put away. Provider stated she would be mindful to ensure sharp kitchen utensils are not left out when children are in care.
Standard 8VAC20-800-320-B
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
The power strip, located on the floor beside the diaper changing table, lacked protective covers in 4 outlets.
Plan of Correction: Outlets will be placed in power strip.
Standard 8VAC20-800-430-A
Children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.
The gate at the entrance to the stairs and the gate to the entrance of the child care area was open when the inspector arrived. There were children in care under the age of two years.
Plan of Correction: Both gates were closed during the inspection. The provider stated she will be mindful to ensure that family members close gates when children are in care.
Standard 8VAC20-800-50-A
Repeat Violation
The family day home shall keep a written record of children in attendance each day.
Child 1, who had been in care for approximately 2 1/2 hours, was not listed on the parent sign-in attendance sheet for the family day home.
Plan of Correction: Child 1 was added to the attendance record during the inspection. The provider stated the parent had failed to sign the child in. The parent will be reminded to sign the child in each morning.
Standard 8VAC20-800-520-A
Cribs shall be provided for children from birth through 12 months of age.
There was only 1 crib in the family day home for 2 children in care under the age of 12 months.
Plan of Correction: The provider stated a second crib had been available however, due to a broken part it was sent back to the manufacturer. A new crib has been ordered and is due for delivery on Wednesday, February 26, 2025.
Standard 8VAC20-800-60-B
Repeat Violation
A child's record shall contain the following required information:
1. The enrollment record for child 1 lacked the following required documentation:
a. Name, address and phone number for a second emergency contact. No address was listed for the first emergency contact;
b. Telephone number for each parent's place of employment;
c. Telephone number for child's physician;
d. Names of persons other than the custodial parents who are authorized to pick up the child;
e. First date of attendance;
f. Parent did not sign enrollment form.
2. The enrollment record for child 2 lacked the following required documentation:
a. Name, address and phone number for a second emergency contact;
b. Work phone number for one parent.
Plan of Correction: Each parent will be asked to provide all missing information. The provider will review all records, prior to enrollment, for completeness going forward.
Standard 8VAC20-800-600-B
Infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour.
Child 2 and child 3 were confined in an infant seat, swing and upright exorciser for periods exceeding 60 minutes. Both children were awake and are under the age of 12 months.
Plan of Correction: Provider stated she would be mindful to arrange schedule and child care area to ensure infants and toddlers have play time on the floor.
Standard 8VAC20-800-610-A
Use of media shall be limited to programs that are produced for children or are suitable for children.
When the inspector arrived a television show, not produced for children, was playing on the television in the child care room. The children in care were awake and able to see and hear this television show.
Plan of Correction: A child centered television show was turned on during the inspection. The provider stated she will be mindful to only turn children's television shows on going forward.
Standard 8VAC20-800-70-B
The provider shall obtain the parent's written acknowledgement of the receipt of the information in the written information for parents.
The parent of child 1 and child 2 did not sign the written information for parents agreement for the FDH.
Plan of Correction: Each parent will be asked to review and sign the parent agreement. Going forward, the provider will check to ensure parent has signed each agreement prior to enrollment of child.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The provider stated that a fire drill had not been conducted in January 2025.
Plan of Correction: The provider stated that a fire drill would be implemented in February and each month thereafter.
Standard 8VAC20-800-830-D
Records of emergency evacuation and shelter-in-place drills shall be maintained for one year.
The provider was not able to locate the record of emergency evacuation and shelter-in-place drills for the family day home.
Plan of Correction: The provider stated there was a written emergency log for the FDH however, it had been misplaced. The provider stated that a new log will be obtained if the old one can not be located.
Standard 8VAC20-800-920
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted or placed in an area accessible to parents.
The menu posted was not for the current one-week period.
Plan of Correction: The provider stated she was working on the menu for the current week. The new menu will be posted.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
6 bottles, for 2 infants in care, were not refrigerated or labeled with the child's name and date.
Plan of Correction: All bottles were refrigerated, labeled and dated during the inspection. Going forward the provider stated she will immediately remove bottles from infant bags and place in refrigerator instead of relying on ice pack placed by parent in carry bag. Additionally, provider will check each bottle to ensure each are dated and labeled with child's name.
