During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 06/23/2025 and completed on 06/23/2025 as part of the conditional licensure period. The on-site inspection began at 10:05 am and completed at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 6. The inspector reviewed 2 children's records on-site. This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the family day home.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/02/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The inspection documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The inspection summary and violation notice from the most recent licensing inspection were not posted in the FDH.
Plan of Correction: New Inspection was posted.
Standard 8VAC20-800-240-A
Systemic Deficiency
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood.
An upright vacuum cleaner was observed in an area of the children care room accessible to children. A large potted plant was placed in the kitchen and child 1 was observed with one of the leaves of the plant in his hand. Hand tools were stored on an outside table accessible to children. The tree stump in the backyard has multiple areas of sharp pointed wood that could cause injury if touched.
Plan of Correction: All items of concern will be removed from area. The room with the plant in it will no longer be used as part of the day care. The stump will be covered with tarp.
All will be completed by July 14, 2025.

Repeat Violation
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, pesticides shall be stored in areas inaccessible to children.
The bathroom cabinet in the bathroom utilized by children was unlocked. Multiple personal care items, deodorizers and pesticides were observed stored in this cabinet.
Plan of Correction: All poisonous substances were put in locked room.
Lock on cabinet was locked on June 20, 2025.
Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.
The landline telephone was not working during the inspection.
Plan of Correction: Cox cable has to send out a person to check the wires. Waiting on an appointment date. Will be corrected as soon as I get an appointment.

Systemic Deficiency
The family day home shall keep a written record of children in attendance each day.
Child 2, who had been in care for approximately 2 1/2 hours, was not listed on the parent sign-in attendance sheet for the family day home.
Plan of Correction: I have signs up instructing parents to sign in and out. Have had a talk with the parent that keep do this. Parent has completed.

Systemic Deficiency
A child's record shall contain the following required information:
1. The enrollment record for child 2 lacked the following:
a. The street address for both emergency contacts;
b. The parent did not sign the enrollment form attesting to the accuracy of the information provided.
Plan of Correction: The correct information was added to child 2 enrollment and parent has signed.
Standard 8VAC20-800-600-B
Repeat Violation
Infants shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib. The intervening time period between confinements shall be at least one hour.
Child 1, age 12 months, was placed in a crib for approximately 50 minutes. Child 1 was awake the entire time.
Plan of Correction: Infants is not put in crib unless it is nap time.

Repeat Violation
Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
The diaper pail in use was hand generated. The diaper pail required the provider to place her hands inside of the pail to dispose of the two wet & soiled diapers changed during the inspection.
Plan of Correction: Has a hand free diaper pail.
Standard 8VAC20-800-920
Repeat Violation
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted or placed in an area accessible to parents.
The menu posted was not for the current one-week period.
Plan of Correction: Menus are now posted on Sunday, to ensure that on Monday it is correct. Correct was posted June 25, 2025.
