Inspector Notes

An unannounced monitoring inspection was initiated on May 6, 2025, and completed on May 17, 2025, in response to a self-report received by the licensing office on May 6, 2025, relating to forbidden action. The inspector reviewed 4 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-410-1
The following actions or threats thereof are forbidden: striking a child. On April 30, 2025, Staff #1, physically disciplined a child by striking the child on her right leg.
Plan of Correction: The center?s plan of correction: An internal investigation was conducted and determined that the incident of forbidden action did occur at the center. The staff member involved was terminated immediately. A formal meeting with the child?s mother was conducted and paperwork was completed and submitted regarding the incident. The owner immediately took preventative measures and conducted mandatory staff meeting re-training to outline the standard, reinforcement of policies on professional conduct and child protection, and regular monitoring and review of staff interactions to uphold commitment to a safe and nurturing environment for children.
