During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on September 3, 2025. The on-site inspection began at 9am and ended at 11am. The inspector reviewed compliance in the areas listed above. There were 19 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-340-C
During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member with direct means for communication between the two of them. There is one staff member present when the first child arrives in the center for childcare at 6:30am. The following staff member does not arrive until 7am.
Plan of Correction: The center responded with the following: The center will adjust the staff member?s schedule to ensure that there are always 2 staff members in the building when one or more children are in care. This will take effect immediately.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present. There was only one list with children?s allergies and dietary restrictions in the kitchen.
Plan of Correction: The center responded with the following: The center will duplicate the list of the children?s allergies, sensitives, and dietary restrictions and put it in each classroom keeping it confidential.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. In reviewing injury reports, the date and time when parents were notified and any future action to prevent recurrence of the injury were not recorded.
Plan of Correction: The center responded with the following: The center will re-do their injury report log and add the required standards to each form. Moving forward, this information will be recorded to ensure that all components of the injury report are recorded and documented.
Standard 8VAC20-780-560-M
No child shall be allowed to drink or eat while walking around. A preschool child was observed eating cereal while standing and walking.
Plan of Correction: The center responded with the following: The center corrected the situation and will ensure that all the children are sitting while eating and drinking.
