An announced, on-site initial inspection was initiated on 09/03/2024 and completed on 09/03/2024. The on-site inspection began at 10:42am and ended at 12:22pm. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 4. The inspector reviewed 1 child record and 1 caregiver record on-site. This inspection included document review and a tour of the family day home.

Based on record review, the provider did not obtain a copy of the results of a search of the central registry check.

Evidence:
The provider did not have documentation for central registry for Caregiver #1, and Household members 1, 2, and 3.
Plan of Correction: Provider had accidentally filed incorrect portability request for the Central Registry Search(CRS). Will submit a request for the CRS today.
