Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 7/8/2025 and completed on 7/8/2025, as a part of the licensure period. The on-site inspection began at 8:45am and ended at 9:55am. The inspector reviewed compliance in the areas listed above. There were five children present and one caregiver; the point total was 14. The inspector reviewed three children?s records and five caregiver/household member(s) records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A person residing in the home who is 14 years of age and older must have a search of the central registry and make the information available for regulatory purposes.

Household member #4 was missing a search of the central registry.
Plan of Correction: A request for a central registry search will be submitted for household member #4.
