During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site focused inspection was initiated and completed on December 2, 2024. The on-site inspection began at 9:30am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There was one child present and one caregiver; the point total was two. The inspector reviewed one children's record and one caregiver record on-site. This inspection included document review, tour of the home, interviews, and observations.

The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided required information in writing. The parents of Child #1 were not provided in writing the required information on whether or not there is liability insurance in force on the family day home operation. The child had been in care for seven weeks.
Plan of Correction: I will provide in writing to the parents today and have them sign the form to be placed in their child's file. I will include this form in all future enrollment packets.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care.

There was no emergency evacuation procedures practiced for October and November.
Plan of Correction: I will conduct a practice evacuation this week. I will conduct an evacuation drill at the beginning of each month.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, documentation of immunization is to be obtained for each child.

Child #1 had been in care for five weeks before documentation of immunizations were received.
Plan of Correction: I will require parents provide their child's immunization documentation on or before the first day for all future enrollments.
