During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on August 19, 2025. The on-site inspection began at 11:30am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was eight. The inspector reviewed two children?s records and one caregiver records on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child #2, who had been in care for over 120 days.
Plan of Correction: Contacted the parents and they will bring me another copy for our records by 8/21/25.
Standard 8VAC20-800-60-B
Children's records are required to contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. The record for Child #2 was missing the written authorization for emergency medical care. The child was present during the inspection and had been in care for a over four months.
Plan of Correction: Contacted the parent and the parent will sign tonight.
Standard 8VAC20-800-80-A
Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child. The file for Child #2, who was present during the inspection and has attended for over four months, did not have documentation the child's identity and age had been verified.
Plan of Correction: Contacted parents and they will bring birth certificate or passport by 8/21/25.

Before attendance, documentation of immunizations is to be obtained for each child.
There was no documentation of immunizations for child #2, who was in care during the inspection and had been in care for over four months.
