During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on October 1, 2025. The on-site inspection began at 3:30 p.m. and ended at 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 15. The inspector reviewed 6 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-800-540-B
Infant walkers shall not be used. Child 2 (age 12 months) was in an infant walker on 10/01/2025.
Plan of Correction: The infant walker was removed and will not be used again.
Standard 8VAC20-800-60-B
.A child's record shall contain the following: written authorization for emergency medical care should an emergency occur and the parent cannot be located. Child 1, Child 2, and Child 3 did not have written authorization for emergency medical care.
Plan of Correction: The parents will be asked to complete the form for emergency medical care.
Standard 8VAC20-800-700-B
The parent's written authorization for medication shall expire or be renewed
after 10 working days. The parent of Child 1 completed a short term written authorization for 3 medications 1 1/2 months ago. The authorizations were not renewed every 10 working days as required.
Plan of Correction: The medications will be returned if a new medication authorization is not received.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain
documentation that the child has been adequately immunized according to the
requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of
Health regulations. Child 5 did not have documentation of an immunization record on file. The child started care 2 days ago.
Plan of Correction: The immunization records will be obtained prior to the child's first day.
