During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 3/20/25, as a part of the conditional licensure period. The on-site inspection began at 11:10am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were four children present (to include one resident child) and two caregivers; the point total was nine. The inspector reviewed three children?s records and two caregiver records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/31/25. A POC submitted after this date will not appear on the public website.
Violations
5A child's record shall contain the all the required information.
The record for Child #2 did not contain the telephone number of one parent?s place of employment, which is required.
The diaper changing table had a cloth cover on it, making it an absorbent surface.
Before the child's first day of attendance, parents shall be provided in writing all the required information.
The records for Child #1 (enrolled for one month), Child #2 (enrolled for five months), and Child #3 did not contain all the required information for parents.
When meals or snacks are provided by the family day home, the menu for the
current one-week period shall be dated.
The menu available for review at the family day home was not dated to show dates for the current week.
