Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/03/24. This licensee is currently on a conditional license. The on-site inspection began at 10:20am and ended at 11:55. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 provider for a total of 10 pts.

The inspector reviewed 5 children?s records and 1 provider record on-site. This inspection included document review, tour of the home. interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Providers are required to have a copy of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.

Records for children #1-#5 did not have copies of the physical examination.
Plan of Correction: The provider will get copies of the physicals for the file. The provider has viewed the physicals but had not retained copies for the file.
Standard 8VAC20-800-350-E
Hot water taps available to children shall be maintained within a range of 105- 120 degrees.

The bathroom faucet that the children use measured 124 degrees.
Plan of Correction: The provider will adjust the temperature.
