During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/18/2024 and completed on 12/19/2024, as a part of the conditional licensure period. The on-site inspection began at 10:10 AM and ended at 11:40 PM. The inspector reviewed compliance in the areas listed above. There were 15 children present and 3 staff. The inspector reviewed 5 children?s records, and 5 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #1, 2 and 3 currently working, and the TB screening was 3 months before starting the position.

Staff # 4 and 5 currently working, and the TB screening was 2 months before starting the position.
Plan of Correction: In the future all TB screenings or test will be completed no more than 30 days prior to hire.
