Inspector Notes

An unannounced inspection was initiated on 02/27/2028 and completed on 03/06/2025 in response to a complaint received by the licensing office on 02/19/2025 relating to allegations of Special Care Provisions and Emergencies, staffing and Physical Plant. The inspector reviewed a staff records on-site. This inspection included document review tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Staff 1 did not properly dispose of fecal matter from Child 1 (age 2.5), while on the playground with other children.
Plan of Correction: Staff will be retrained on proper toileting.
