During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 5/1/2025 and completed on 5/9/2025) in response to a self-report received by the licensing office on 4/29/2025 relating to supervision. The inspector reviewed 1 child?s record and 2 staff records on-site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/16/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom. Depending on the location and layout of the restroom, staff may need to provide intermittent sight supervision of the children in the restroom area to assure the safety of children and to provide assistance.

On 4/28/2025, Child 1(age 3 years) entered the playground bathroom, located across the street from the center, unnoticed by staff. Staff and all children proceeded to exit the playground to return to the center. Staff locked the bathroom doors on the playground, shut and locked the playground gate, and crossed the street to take the class inside the center building. During transition, it was noticed Child 1 was not with the class. Staff 2 went back to the playground to check for the child. Upon entering the gate Staff 2 could hear Child 1 crying in the bathroom. Staff unlocked the door and found Child 1 alone in the bathroom. Child 1 had been in the bathroom approximately 4-7 minutes.
Plan of Correction: Staff members were retrained on the center's supervision policies and procedures on 4/29/2025.

Centers shall be in compliance with the center's own policies and procedures required by these standards.

The center?s policy is that children must be in sight and sound of center staff at all times so they can protect the health and safety of the children. Child 1 (age 3 years) was not in the sight or sound of center staff on 4/28/2025. Child 1 was out of sight and sound for approximately 4-7 minutes.
Plan of Correction: New policy and procedures were written regarding the use of playground bathrooms on 4/29/2025.

Staff was trained regarding new procedures and signed acknowledgement forms.
