Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 08/26/2025 and completed on 08/26/2025. The on-site inspection began at 1:00 pm and ended at 3:15 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The criminal background check for Staff 4 was dated 3 days after the date of employment.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child as it occurs.

The departure of all the children in Classroom 2 was not documented on 08/25/2025. The arrival of all the children in Classroom 2 was not properly documented on 08/26/2025.
Plan of Correction: Will review the attendance procedures during professional development and will help remind the staff to ensure attendance is accurate.
