During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 03/12/2026. The on-site inspection began at 10:30 am and ended at 1:05 pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
5Providers must obtain a completed national criminal background check prior to the employee?s first day of employment.
The national criminal background check was completed 5 days after the date of employment.
The tuberculosis screening for Staff 4 was dated 3 days after the date of employment.
The children, ages 3 years-4 years, were served coin shaped carrots slices and string cheese for lunch; pieces of hard raw vegetables and large chunks of cheese are considered to be potential choking hazards.
Child 2 (age 4 years) has a diagnosed food allergy; there was not a written care plan available for Child 2.
The records for Staff 3 and Staff 4 did not contain all of the required components.
