During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 7/30/2025. The inspection started at 10 AM and concluded at 10:40 AM. There were 5 children present and 2 caregivers; the point total was 12. The inspector reviewed 3 children?s records and 2 caregiver/household member records. This inspection included: document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), a tour of the program, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/6/2025.

Electrical outlets shall have protective covers.

There were 6 electrical outlets observed in the child care area without protective covers.

If hazardous substances are not kept in original containers, the substitute container shall clearly indicate their contents.

An unlabeled spray bottle was observed on a shelf on the wall. The provider confirmed that the bottle contained a mixed bleach-water solution.

The vendor shall maintain documentation of the current rabies vaccination for dogs.

The rabies vaccination for two of the family dogs expired 5/18/2025. The provider confirmed that the vaccination was expired.
