Inspector Notes

An unannounced, on-site renewal inspection was completed on 04/30/2025. The on-site inspection began at 2:33 p.m. and ended at 4:55 p.m. The inspector reviewed compliance in the areas listed above. There were 13 children present and 2 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/21/2025. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunizations for C1 were not dated. The licensing inspector was unable to determine when they were received by the center.

The required ratio for children aged 2 is 1 staff per 8 children. S1 was observed by licensing inspector leaving the classroom to put away nap blanks leaving S2 in the classroom with 12 children and the youngest child in the group was 2 years old.
