During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 04/01/2026. The on-site inspection began at 10:30 a.m. and ended at 1:46 p.m. The inspector reviewed compliance in the areas listed above. There were 18 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/15/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled
that contains all of the required information. The center did not have records for C4 and C5 enrolled for 3 days.
Plan of Correction: The center Director will maintain accurate information on every child before they start. The files will contain Information that is required to complete the child?s file and meet licensing standards. Each child?s will contain a check-off list to verified the information have been received.
Standard 8VAC20-780-80-A
The center shall maintain a record of daily attendance that documents the arrival and
departure of each child in care as it occurs. Upon arrival at the center, licensing inspector observed 18 children present. The daily attendance record documented 15 children signed in for the day.
Plan of Correction: It is important that accurate attendance is kept to verify who is present or absent. Also to verify where a child is located. The opening staff will assist parents with signing his/her child in and out each day. The staff will serv as a witness to whom drop the child off and sign the child in, and who picked the child up and signed them out. The Signed In/Out book, Attendance, and Head Count, will be checked throughout the day by the Director. The Director will express to the parents and staff the importance of this procedure. Assuring a safe environment for our children is one of our main goals.
