During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on March 17, 2025 and completed on March 17, 2025 , as a part of the current licensure period. The on-site inspection began at 9:00am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 6. The inspector reviewed 2 children?s records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Violations
8The current licensing inspection notice was not posted on the premises.
The program did not have evidence of a testing plan submitted to VDH and the program did not have evidence of the implementation of the required testing plan.
~A brick walkway was observed on the side of the outdoor play area that had several unleveled bricks, posing a tripping hazard.
~Hazardous materials were observed in the outdoor play area, accessible to children, that included, a ladder, scaffolding approximately 6 ft high and 4 ft wide, 5 gallon chemical spray bottle, and tools.
The hot water in the bathroom available to children had a temperature of 155 degrees.
1. Not be installed over concrete, asphalt, or any other hard surface;
2. Be placed at least six feet from the perimeter of other play structures or
obstacles; and
3. Be firmly anchored with ground supports that are covered with materials to
protect children from injury.
The outdoor play structure (over 4 ft high) is noted to be on the covered patio, within 3 feet of the wall of the home, on a hard surface without adequate ground support.
There was no documentation of the parents being notified of the emergency procedures in child files #1 and 2, both in care for over one month.
The responsible person for emergency back-up care.

The telephone numbers for an emergency back-up care provider and the poison control were not posted.
1. Includes emergency evacuation, emergency relocation, and shelter-in-place
procedures;
2. Addresses the most likely to occur scenarios, including but not limited to fire,
severe storms, flooding, tornadoes, and loss of utilities; and
3. Includes provisions for a responsible person who is 18 years of age or older
and is able to arrive at the family day home within 10 minutes for emergency
backup care until the children can be picked up by their parents.
There is not a written plan available for review. The provider states that she was unaware that the emergency preparedness and response plan needed to be in writing.
