During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on March 10, 2026 and completed on March 10, 2026, as a part of current licensure period. The on-site inspection began at 9:20am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 23. The inspector reviewed 7 children?s records and 6 caregiver/household member records. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 18, 2026. A POC submitted after this date will not appear on the public website.
Violations
12The sworn statement for Caregiver 1, who is currently working, was dated 10 days after their first day of employment.
Caregiver #1, who was actively working at the FDH, had a national criminal background check dated 14 days after first day of employment.
Caregiver #1 did not have record of a central registry search results being requested and had been employed for over 45 days. Caregiver #1 was working alone with children.
documentation of education, name address and telephone number of a person to be notified in an emergency and that two or more references were checked before employment.
Caregiver #1, who has been employed for over 45 days did not have record of an emergency contact, documentation of education, and that two or more references were checked prior to date of hire.
Caregiver #1 had record of orientation completed 48 days after date of hire.
Toys available to children in the play area were small enough to pose a choking hazard.
A room that was not disclosed to Licensing as part of the daycare, was being used for childcare.
~Caregiver 1 was left alone with children (23 points) during inspection for a period of approximately 30 minutes.
~Child E was left unattended in another room, seated in a bouncy chair with the door closed, for a period of approximately 20 minutes. Caregiver 1 stated that child had been sleeping.
From 9:20 am until 9:50am, Caregiver #1 exceeded 16 points. There were 7 children, totaling 23 points with Caregiver #1 until 9:50am when provider came from upstairs.
Child E was not moved to the crib after falling asleep in the bouncy chair.
Caregiver #1 did not wash their hands after wiping multiple children's nose.
The only drill documented in the past year was on 2/10/26.
