During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on March 10, 2026 and completed on March 10, 2026, as a part of current licensure period. The on-site inspection began at 9:20am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 23. The inspector reviewed 7 children?s records and 6 caregiver/household member records. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 18, 2026. A POC submitted after this date will not appear on the public website.

The provider is required to obtain a completed sworn statement prior to the employee's first day of employment.

A caregiver shall be physically present on site and provide direct care, to include awareness of and responsibility for each child in care, including being near enough to intervene if needed, and the monitoring of each sleeping infant.

~Caregiver 1 was left alone with children (23 points) during inspection for a period of approximately 30 minutes.
~Child E was left unattended in another room, seated in a bouncy chair with the door closed, for a period of approximately 20 minutes. Caregiver 1 stated that child had been sleeping.

From 9:20 am until 9:50am, Caregiver #1 exceeded 16 points. There were 7 children, totaling 23 points with Caregiver #1 until 9:50am when provider came from upstairs.
