During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on February 5, 2025. The on-site inspection began at 3:30 pm and ended at 4:35 pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 4 staff. The inspector reviewed 5 children?s records on-site and 4 staff records electronically on February 6, 2025. This inspection included document review, observations, interviews, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/14/2025. A POC submitted after this date will not appear on the public website.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #3, who is working with children, has not yet completed this course which is overdue by 2 months.

Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member or independent contractor that has been trained in Medication administration.

1. Child #7 and child #8 was in care with staff and requires emergency medication. However, staff does not have medication administration training.
2. Staff #2 confirmed that staff did not have medication administration training.
