Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) completed on 7/8/2025. The inspection started at 9:40 am and concluded at 10:25 am. There were no children present, but 4 children enrolled and (1) caregiver. The inspector reviewed (4) children?s records and (#5) caregiver/household members records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
1Child # 1, #2, #3 and #4 did not have documentation of proof of birth. All should have attended for over 10 months.
