During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 4/14/2025, as a part of the conditional licensure period. The on-site inspection began at 11:00 AM and ended at 12:45 PM. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff/volunteer records on site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Repeat violation
Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after attending.
Child 5 has attended over five months without providing documentation of a physical examination by or under the direction of a physician.
Plan of Correction: The parent of child 4 will be required to obtain a physical for the child or the child will not longer be allowed to attend.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The written record shall include all required documentation.
An injury record for child 5 did not include the date the parent was notified of the injury and was signed by only one staff person.
Plan of Correction: All injury reports will be printed as a hard copy to be kept in the child's records and will include the date, time and how the parent was notified of the injury and will include either two staff signatures or the signature of one staff person and the parent.
