During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/22/25. The on-site inspection began at 10:00am and ended at 11:47am. The inspector reviewed compliance in the areas listed above. There were 39 children present and 10 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-260-B
Annual approval from the health department shall be provided. The annual health inspection was expired by 3 months.
Plan of Correction: The center responded with the following: The health department was contacted to schedule the health inspection.
Standard 8VAC20-780-280-B
Hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. In the two's classroom, there was a can of air freshener sitting on the desk and a container of couch cleaner/stain removernin an unlocked cabinet under the sink.
Plan of Correction: The center responded with the following: The cleaning materials were removed and placed in a locked area during the inspection. Staff will be reminded to lock all hazardous chemicals when not in use.
Standard 8VAC20-780-570-A
When feeding tables are used the protective belt shall be fastened securely. In the junior toddler classroom, there were five children sitting at a feeding table without protective belts.
Plan of Correction: The center responded with the following: The feeding table was given to the center and the straps were not included with the table. Protective belts for the table have been ordered.
Standard 8VAC20-780-60-A
The center shall maintain and keep at the center a separate record for each child enrolled that contains all the required information. The records for child 1 (enrolled for 6 months) and child 2 (enrolled for over 1 month) do not contain the addresses for the two emergency contacts.
