Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 6/16/25, as a part of the conditional licensure period. The on-site inspection began at 11:45am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were no children present and one caregiver. There are currently, no children enrolled. This inspection included document review, tour of the facility, interviews and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
