A child day center operated or conducted under the auspices of a religious institution, shall post the fact that it is exempt from licensure in a visible location and post the qualifications of the personnel employed.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.

The center shall establish and implement procedures ensuring that all children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.

Child #1, child #2, child #3, child #4 and child #5, currently enrolled at the time of inspection, did not have documentation of receiving immunizations.

Systemic Deficiency:
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

Staff #1, employed for 11 months, did not have a completed sworn statement available to view at the time of inspection. Staff # 2, employed for six months, did not have a completed sworn statement available to view at the time of inspection. Staff # 4, employed for six months, did not have a completed sworn statement available to view at the time of inspection.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Systemic Deficiency:
Upon enrollment of a child in a regulated child day program, such child day program shall require information from the person enrolling the child regarding previous child day care and schools attended by the child, and proof of the child's identity and age.

The record for child #1 and child #3 did not contain proof of the child's identity and age. The record for child #1, child #2, child #3, child #4 and child #5 did not contain previous child day care and schools attended by the child.
