For each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Four of 12 children that were present at the time of inspection were not checked in.

Systemic Deficiency:

The child's record shall include all required information.

The record for child #1 was missing an address for the parent, and an address for an emergency contact. The record for child #2 was missing one emergency contact, the child?s address. The record for child #4 was missing address and phone number for one parent and parent work. The record for child #5 was missing phone and address of parent work.

Systemic Deficiency:

A written statement is required that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible.

The record for child #1, child #3, child #4 and child #5 did not have the written statement signed by the parent.

Systemic Deficiency:

The following records shall be kept for each staff person.

The record for staff #1, employed for 11 months, did not contain verification of age. The record for staff #2, employed for six months, did not contain verification of age. The record for staff #4, employed for six months, did not contain verification of age.

Repeat Violation & Systemic Deficiency:

Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.

Staff #1, employed for 11 months, did not have a complete TB screening on file at the time of inspection. Staff #2, employed for six months, did not have a complete TB screening on file at the time of inspection. Staff # 4, employed for six months, did not have a complete TB screening on file at the time of inspection.

Systemic Deficiency:
Within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

Staff #2, employed for six months, did not have a completed Virginia Preservice Training for Child Care Staff on file. Staff #3, employed for one year, did not have a completed Virginia Preservice Training for Child Care Staff on file. Staff #4, employed for six months, did not have a completed Virginia Preservice Training for Child Care Staff on file.

Systemic Deficiency:
Orientation training for staff shall be completed on the following facility specific topics prior to the staff member working alone with children and within seven days of the date of employment.

Staff #1, employed for 11 months, did not have a complete orientation training on file. Staff #2, employed for 6 months, did not have a complete orientation training on file. Staff #3, employed for one year, did not have a complete orientation training on file. Staff #4, employed for six months, did not have a complete orientation training on file.

All staff who work directly with children shall have within 90 days of the date of employment a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Staff #3, employed for over one year, did not have current certification on record. Staff #4, employed for six months, did not have current certification on record.

Staff who work directly with children shall, in addition to preservice and orientation training required, annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.

Staff #3, employed for over one year, did not have any training documented at the time of inspection.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

In the main classroom, an outlet strip had no covers, and outlets on the wall were not covered.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Cleaning and hazardous materials were observed in an unlocked closet located in the main room where children were present.

Disposable diapers shall be disposed in a leak-proof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

In the classroom where diaper changing occurs, there was no foot operated trashcan or storage system.

Evacuation procedures shall be practiced at least monthly.

There were no documented evacuation drills at the time of inspection.
