During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated on 5-19-2025 and completed on 5-22-2025. The onsite inspection began at 5:25pm and ended at 6:15pm. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 staff. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/30/2025.

Violations
10A child day program that is licensed and serves preschool-age children, shall be equipped with at least one carbon monoxide detector. There was not a carbon monoxide detector in the center.
The areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. In the multipurpose room and hallway there were metal electrical outlet plates that were protruding from the wall with sharp edges.
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Hand sanitizer was observed on a shelf in the lobby and a fragrance spray was observed in the snack room.
There shall be a posted daily schedule for preschoolers that allows for flexibility as children's needs require. The daily scheduled posted ended at 5:00pm and Child #1 was observed in care at 5:25pm. The center closes at 8:00pm. The daily schedule does not include all the centers hours of operation.
A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone. These emergency phone numbers were not posted in a visible place.
