Inspector Notes

An announced, on-site Initial inspection was initiated and completed on September 12, 2024. The on-site inspection began at 9:00 AM and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were no children present and two staff. The inspector reviewed two staff records on-site. This inspection included document review, tour of the facility, interviews, and
Measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on record review, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and shall have been completed within 30 calendar days of the date of employment.

Evidence:
The record of staff #2, employed 08/21/2024, did not contain the results of a tuberculosis screening.
Plan of Correction: Per the owner: I will submit our TB results on 09/13/2024. Going forward, we will make sure staff have their TB test prior to hire date.
