During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 23, 2026, and completed on March 23, 2026, as a part of the licensure period. The on-site inspection began at 9:00 am and ended at 10:52 am. The inspector reviewed compliance in the areas listed above. There were 47 children present and eight staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The record of staff #1, employed for three months, did not have documentation of a criminal history being requested prior to employment.
Plan of Correction: Per the owner: We will ensure all items are collected in the time required.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. The record of staff #3 contained documentation of completing the Virginia Department of Education-sponsored orientation course nine months after employment.
Plan of Correction: Per the owner: I will ensure it is completed and in the staff's folder.
Standard 8VAC20-780-510-F
The center shall ensure the medication authorization is available to staff during the entire time it is effective. Two medications for child #6 did not have written parent authorizations.
Plan of Correction: Per the owner: We will ensure all forms are current and updated.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days, or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication authorization for child # 7 expired two months and 24 days ago. The medication had not been disposed of and was on site.

Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill. The center did not have documentation of conducting an evacuation drill for the month of February 2026.
Plan of Correction: Per the owner: I will ensure fire drills are documented.
Standard 8VAC20-780-70
Staff records shall be kept for each staff person and contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required. The record of staff #1, identified as a program lead and observed alone with a group of children, contained 18 hours of training when 24 hours was required.
