The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Staff #1 was dated nine days after their first day of employment. The sworn statement for Staff #2 was dated 4 days after their first day of employment.
Plan of Correction: The director will review and sign off on all onboarding paperwork prior to employee first scheduled shift.

The diapering surface shall be used only for diapering or cleaning children. There were several items sitting on top of the diapering surface in one of the classrooms.
Plan of Correction: All items were received immediately. All staff has been retrained on proper procedures and only be used for diapering.
