During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/5/2025, as a part of the licensure period. The on-site inspection began at 10:30 AM and ended at 11:33 AM. The inspector reviewed compliance in the areas listed above. There were 13 children present and 4 staff. The inspector reviewed 3 children?s records and 2 staff records on-site
This inspection included: document review, tour of the facility, interviews and observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/14/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center maintains compliance with the center's own policies and procedures that are required by these standards.
The center's medication policy did not include methods to prevent the use of outdated medication.
Plan of Correction: CHMS never administers expired medication. CHMS updated its written policy to reflect actual practices. Families acknowledged and signed.
Standard 8VAC20-780-540-C
The required first aid kits shall include at a minimum, triangular bandages.
Both first aid kits had only one triangular bandage.
Plan of Correction: An additional triangular Bandage was added to each first aid kit on each floor.
