Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated due to a relocation of the SHSI program on 8/27/2025 and completed on 8/27/2025. The inspection started at 11:20 am and concluded at 12:00 pm. There were 3 children present and 1 caregivers; the point total was 6. The inspector reviewed 2 children?s records and 2 caregiver/household members records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
12 of 2 children were missing the home address and telephone number for each parent who has custody.
2 of 2 children were missing 1 person designated by the parent to contact in case of emergency if the parent cannot be reached.
