Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 8/12/2025. The inspection started at 10:35 am and concluded at 11:40 am. There were no children present and 1 caregiver. The inspector reviewed 5 children?s records and 1 caregiver/household member(s) records.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The vendor / provider did not have a written record of daily attendance for children in care for 8/11/2025 as they arrive and depart the program.
Plan of Correction: Now I will use that form.
Standard 8VAC20-790-190-B
There was missing required information in the child's record.

Child #1 does not have information for emergency contact ( name, address and telephone number).
Plan of Correction: As soon as mommy comes I will ask for emergency contact.

The diapering surface shall be nonabsorbent and washable.

The diaper changing pad had a tear and was no longer nonabsorbent.
Plan of Correction: I will buy new one.
Standard 8VAC20-790-430-B
The vendor shall maintain a record of the dates of the practice drills for one year.

The provider / vendor completed but could not locate drill log documenting evacuation drills for January, February, March, April, May, June and July 2025.
Plan of Correction: The log was full. We did it but I can't find it.
