The vendor shall maintain and keep at the center a record for each enrolled child, which shall be made accessible to the departments representative upon request.
The following information was missing for two children (child #1 and child #2), who had enrolled approximately 6 months ago.
1.The home address, and telephone number for each parent who has custody
2.Name, address, and telephone number for each custodial parents place of employment or school attendance.
3.The address for at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached
