Inspector Notes

An announced, on-site initial inspection was completed on 09/25/2024, as a part of the licensure period. The on-site inspection began at 9:55 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were no children present. The inspector reviewed a mock children?s record and 6 caregiver/household member's records. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/09/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include chipped or peeling paint.

The ceiling in the playroom had a large patch of peeling and cracked paint.
Plan of Correction: We will sand and repaint the ceiling in the playroom.
Standard 8VAC20-800-430-D
Protective barriers for stairs shall be constructed to prevent a child from climbing over, crawling or falling through, or becoming entrapped.

The protective gate at the top of the outdoor stairs leading to the basement left the top two stairs accessible. There was a gap under the bottom of the gate to the third stair of 10 inches. There was a gap in the vertical bars on the gate that was greater than 4 inches.
Plan of Correction: We will extend the guardrail for the staircase out, so that the gate is at the top of the stairs with no gap underneath. We will also adjust the opening in the gate so that the gap in the vertical bars is less than 4 inches.
Standard 8VAC20-800-500-B
The highest climbing platform on outdoor climbing equipment shall not exceed four feet for preschool children.

The outdoor climbing structure was 5 feet from the ground.
Plan of Correction: Preschool age children will not use the play structure. We will also move the play structure away from the main outdoor play area.
Standard 8VAC20-800-500-C
Stationary outdoor playground equipment shall be placed at least six feet from the perimeter of other play structures or obstacles.

The stationary outdoor play structure did not have six feet of clearance around all sides. One side of the structure was only 3 feet from the side of the house, and less than a foot of space from the guardrail of the outdoor stairs.
Plan of Correction: We will move the outdoor play structure away from the side of the house and ensure there is 6 feet of clearance around all sides.
