A trained person will conduct daily simple health screenings and exclusion of children.

No staff have daily health training.
Plan of Correction: We updated the teachers handbook for 2026/2027 to ensure teachers submit all required documentation prior to their start date. Two of the staff too the training. The rest of the teachers will finish it by 12/1/2025.
Standard 22.1-289.031-B-4
The center is required to obtain updated immunizations prior to the child's first day of care.

Child #1's immunization were obtained 17 days after their first day of care.
Plan of Correction: We updated the students handbook for 2026/2027 to ensure teachers submit all required documentation prior to their start date. All immunizations for all students now are there on their files.

-The sworn statement for Staff #1, who is currently working, was dated 20 days after their first day of employment.

-The sworn statement for Staff #2, who is currently working, was dated 28 days after their first day of employment.

Plan of Correction: We updated the teachers handbook for 2026/2027 to ensure teachers submit all required documentation prior to their start date. Also we submitted an appointment for them and will have this completed soon.

-Staff #2, who had been employed for three months, did not have a central registry search requested.
Plan of Correction: We updated the teachers handbook for 2026/2027 to ensure teachers submit all required documentation prior to their start date. The request have been submitted and we are waiting for the results.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

-Staff #2, employed for more than three months, did not have documentation of requesting a central registry check, criminal history check, and sex offender registry from one state.
Plan of Correction: We updated the teachers handbook for 2026/2027 to ensure teachers submit all required documentation prior to their start date. An appointment has been book for the teacher.
