During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 9/4/2025. The on-site inspection began at 12:00pm and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 1 caregiver. The inspector reviewed 3 children?s record and 1 caregiver/household member records on-site.

This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/15/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance or within 30 days after the first day of attendance.

Child B enrolled for 4 months, did not have documentation for a physical examination on file.

Standard 8VAC20-800-110-B
The provider shall obtain documentation signed by a licensed physician, the physician's designee, or an official of a local health department that includes dates immunizations were received for all children by date of enrollment.

Child C enrolled for 2 months, had immunization records on file but they were not signed or stamped by a licensed physician, the physician?s designee, or an official of a local health department.

No soft bedding of any kind shall be used under or around infants.

Child A (7 months) was observed in a car seat sleeping with a blanket across his lap.

Infant carrier seats shall be used according to the manufacturer?s instructions and when occupied by a child.

Child A (7 months) was observed sleeping in an infant carrier seat for approximately 4 minutes. The provider stated that it is the only way the child will sleep.
