During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on March 25, 2025 and completed on March 25, 2025, as a part of the conditional licensure period. The on-site inspection began at 1:35 PM and ended at 3:10 PM. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 26. The inspector reviewed five children?s records and four caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 4, 2025. A POC submitted after this date will not appear on the public website.

11The record for caregiver #3, did not have a sworn statement and has been employed for 85 days.
Caregiver #3, who had been employed for 85 days, did not have a completed national criminal background check.
The record for caregiver #3, who had been employed for 82 days did not have a central registry search check. On the day of the inspection, caregiver #3 was working with children.
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
There were no physical examination records available for child #2, enrolled for 165 days, child #3, enrolled for 165 days, child #4, enrolled for 165 days, and child #5, enrolled for 113 days.
There was no record for caregiver #2, who has been providing care for 82 days.
In the bathroom used by children in care, potentially poisonous cleaning chemicals were on the floor and on the sink counter accessible to children.
The provider shall ensure that hot water at taps available to children shall be maintained within a range of 105 degrees Fahrenheit to 120 degrees Fahrenheit .
The sink in the restroom designated for children, had hot water temperatures that measured at 123.3 degrees Fahrenheit.
Child #3 who is under two years of age was observed napping in a crib using a boppy pillow under their head and back.
Child #1 was observed sleeping in a play pen. The provider acknowledged that child #1 was using the play pen as a sleeping area.
The records for child #1, enrolled for 165 days, child #2, enrolled for 165 days, child #3, enrolled for 165 days, child #4, enrolled 165 days, and child #5, enrolled 113 days did not have written acknowledgement of policies for the administration of medication and provisions of the emergency preparedness and response plan.
The provider shall ensure that the emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
There was no documentation that emergency evacuation procedures had been practiced for the month of February 2025.
