During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on June 2, 2025 and completed on June 10, 2025 with a local agency in response to a self-report received by the licensing office on May 29, 2025 relating to forbidden actions. The inspector reviewed one child?s record and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to staff records and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 8, 2025. POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with the children.

The record for staff #1 did not have documentation of a completed TB screening for Staff #1. The TB screening was 28 days overdue.
Plan of Correction: Staff member had submitted a receipt from TB screening, but not results. Director will audit to ensure appropriate documentation of staff files requirements.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. Staff #2 stated that on 5/20/2025 child #1 (age 2 years) had a toileting accident on the playground. Staff #2 stated that staff #1 called child #1 over to her. Staff #2 asked the child, did you poop? Child #1 said yes and staff #1 shoved child #1 causing him to fall backwards to the ground. Child #1 began to cry.

2. On 5/20/2025 video footage showed, staff #2 observing staff #1 shove child #1 (age 2 years) down to the ground. Staff #2 did not respond to the child who was crying from being pushed down to the ground by staff #1.
Plan of Correction: Staff #2 reported her concerns with staff #1 to the Director. Director coached staff #2 on how to respond in these incidents and required ALL staff to complete an updated mandated reporter training.
Staff #2 was given disciplinary action to ensure safety of children in the future.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards and the center's own policies and procedures that are required by these standards.

1. The center's discipline policy states that: it is our policy to use the principles of reinforcement and to avoid punishment when working with our students. The following actions or threats thereof are forbidden: physical punishment, striking a child, and punishment for toileting accidents.
2. On 5/20/2025, staff #1 placed her hand on child #1?s (age 2 years) forehead shoving child #1 to the ground after learning that child #1 had a toileting accident.
Plan of Correction: Director immediately terminated employment of the staff member after seeing video footage of the incident and self-reported to both licensing and CPS. Director informed all parents of the incident in full transparency. Physical violence is strictly prohibited.
Standard 8VAC20-780-410-1
The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.

Plan of Correction: Director immediately terminated employment of the staff member after seeing video footage of the incident and self-reported to both licensing and CPS. Director informed all parents of the incident in full transparency.
