***Repeat Violation***
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #5, employed for 15 days, had documentation of requesting a central registry two days after employment.

Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required shall be in each staff record.

Staff #5 is designated as Program Leaders and does not have documentation of qualifications.

***Repeat Violation***
Each staff member shall submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with the children.

The TB screening for staff #6 was completed over 480 days prior to hire.

Hazardous substances must be in a locked place that prevents access by children.

In the bathroom #2 located in the playroom area, one container of air freshener was on a shelf and not kept in a locked place.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Child #3 had one medication which expired on 9/25. The medication was not sent home or disposed of.
