Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 02/04/2025 and completed on 02/04/2025, as a part of the conditional licensure period. The on-site inspection began at 8:25 a.m. and ended at 10:15 a.m. The inspector reviewed compliance in the areas listed above. There were 12 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Medication shall be labeled with child?s name, the name of the medication and dosage amount, and time to times to be given.

Medication 1 was not labeled with child?s name.
Plan of Correction: Medication was labeled during inspection.
Standard 8VAC20-780-510-L
Medication, except for those prescriptions designated otherwise by written physician?s order, including refrigerated medication and staff personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.

Medication 1 was in an unlocked storage container on the shelf of the changing table.

The center shall implement a monthly practice evacuation drill.

The was no documentation of a monthly practice evacuation drill for December 2024 or January 2025.
