During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on 04/30/3035 and completed on 05/06/2025. The on-site inspection began at 9:15 a.m. and ended at 11:35 a.m. The inspector reviewed compliance in the areas listed above. There were 17 children present and 5 staff. The inspector reviewed 5 children?s records on-site and 5 staff records and 1 agent/applicant record electronically on 05/06/2025. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-350-C
When children are in ongoing mixed age groups, the staff to child ratio and group size applicable to the youngest child in the group shall apply to the entire group.

On 04/30/2025, for 15 mins, Staff 2 was the only staff member supervising 6 children, ages 12 months to 2 years. The required staff ratio for this group of children was 1:4. Two staff would be required.
Plan of Correction: Provider will ensure they maintain child to adult ratios.

Infant walkers shall not be used.

Child 1, 12 months old, was observed in an infant walker.
Plan of Correction: Walkers were removed during the inspection.

Tables and high chairs shall be sanitized before and after each use for feeding.

Table were not sanitized before the morning snack.
Plan of Correction: Staff will sanitize before and after use for meals/snacks.

Prepared infant formula shall be refrigerated, dated and labeled with child?s name.

There were three bottles of formula with no date or name located on top of the refrigerator in the infant room.
Plan of Correction: Bottles were labeled with name and date during the inspection.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The records for Child 3, Child 4 and Child 5 enrolled over three months, were each missing several of the required components.
Plan of Correction: Provider obtained info from parents.
