During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 02/24/2025 and completed on 02/24/2025, as a part of the licensure period. The on-site inspection began at 1:15pm and ended at 4:25pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and eight staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The immunizations for Child #1 were obtained four days late and the immunizations for Child #3 were obtained two days late.
Plan of Correction: Moving forward, the center will ensure immunizations and physical examinations are obtained within the required time frame.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

The physical examination in the record of Child #1 was obtained two days late.
Plan of Correction: Moving forward, the center will ensure immunizations and physical examinations are obtained within the required time frame.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

The updated TB screening for Staff #5 was completed 17 days late.
Plan of Correction: Moving forward, the center will ensure there is no gap between TB screenings.
Standard 8VAC20-780-550-K
A document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business such as field trips or pick up or drop off of children to or from schools is required to be kept in vehicles used to transport children to and from the center.

The center does not have the required documentation in the vehicle used to transport children.
Plan of Correction: The director will work on the document and have it completed by the following Friday.

Any vehicle used by the center for the transportation of children is required to meet the safety standards set by the Department of Motor Vehicles.

The State inspection sticker in the vehicle used for transportation of children expired over a month ago.
Plan of Correction: The vehicle was taken for an inspection on 02/25/25.
